Why Erbium:YAG Lasers Offer Better Results for Skin Resurfacing Than CO2 Lasers

1. An Erbium:YAG laser wavelength is optimal for skin ablation.
Erbium:YAG lasers have a very efficient beam to target water and relatively limited hemoglobin absorption. The 2,940nm unique wavelength of Erbium:YAG delivers an almost instant skin ablation while limiting heat damage in the surrounding area.
This thermal damage is the biggest difference between Erbium:YAG and Carbon Dioxide (CO2) lasers. Because the absorption in skin tissue is relatively low in CO2 lasers compared to Erbium: YAG lasers (in both water and hemoglobin), their result is substantial excess heat in the surrounding tissue. The consequences of that are localized tissue death, more post-treatment discomfort, and greater instances of post-inflammatory hyperpigmentation (PIH, dark spots) in CO2 lasers.
2. Erbium:YAG laser treatments create less thermal damage and are less painful.
Thanks to the greatly limited thermal effects of Erbium lasers, Erbium:YAG treatments are notably less painful than CO2 lasers, and patients do not require any anesthesia when receiving treatment for small lesions. These safety features make Erbium:YAG lasers the superior choice for treating delicate areas such as the periorbital region, neck, chest, and hands.
3. Erbium:YAG laser treatments have a lower risk of side effects, including hyperpigmentation and/or skin bleaching, compared to CO2 treatments.
Hyperpigmentation and hypopigmentation are mainly due to thermal damage. Because the absorption in skin tissue is relatively low in CO2 lasers compared to Erbium: YAG lasers (in both water and hemoglobin), their result is substantial excess heat in the surrounding tissue. Excessive heating in surrounding tissues can result in either skin darkening (hyperpigmentation) or skin bleaching (hypopigmentation), which more likely can be a side effect of CO2 lasers.

Erbium:YAG lasers require a shorter recovery time compared to CO2 lasers because of the shallow depths of wounds they create. Furthermore, Erbium:YAG lasers deliver reduced thermal damage to the surrounding skin tissue, allowing for much faster healing compared to the deeper injuries and extensive thermal damage that CO2 lasers produce.

WHAT IS THE MOST SUITABLE FOR ME? Sofwave™, Ultherapy™ OR Thermage FLX™?

As we age, there are generally 3 categories of ageing styles we fall into, with each ageing style exhibiting distinct ageing traits. While we will almost certainly have a mix of features, this is a useful guide to explain how different treatments may be more suitable for different individuals.

The 3 aging styles are: “The Sinker”, “The Sagger” and “The Wrinkler“.

“Sinkers” are characterized by volume loss around key areas of the face such as the temples, mid-face and cheeks. This can cause a sunken, “hollowed-out” or gaunt appearance. Between Thermage, Sofwave and Ultherapy, Sofwave and Thermage are more suitable for sinkers as they do not melt fats and accelerate the gaunt appearance. Sinkers may also see more benefit with a combination of skin tightening treatments including dermal fillers to combat the volume loss.

“Saggers” commonly complain of loose skin and tissue. This is due to the loss of elasticity in addition to collagen. Ultherapy is useful to stimulate skin tightening of the SMAS layer to achieve a deeper lifting effect. Sofwave, another Ultrasound technology device, also FDA-approved for skin lifting, is also an appropriate treatment for those saggers who do not want to have the downtime that accompanies Ultherapy.

“Wrinklers” have predominantly wrinkly skin, often located around the eyes, brows and lips. Botox would be able to achieve nice, immediately-visible results for these, but for a longer-lasting treatment, Thermage, Sofwave and Ultherapy can be used. A person with multiple wrinkles and fine lines on the brows, around the eyes and face may benefit more from  Sofwave or Thermage, while one with wrinkles and also saggy skin may be more suitable for Sofwave or Ultherapy.

Also we should take into consideration the pigmentations or propensity to have pigmentations of the patient (whether the patient has freckles, melasma or dark birthmarks) on the face. Ultrasound technology such as Sofwave and Ultherapy would be more appropriate for the patient vs Thermage.
